Exercise and Brain Health: The Strongest Evidence in Cognitive Wellness

If cognitive wellness had a single best-evidenced intervention, this would be it. And it costs nothing.

The short answer

Regular aerobic exercise has the strongest evidence base of any intervention for cognitive health in ageing. Randomised trials have linked it to increased hippocampal volume, improved executive function and better cerebral perfusion, with mechanisms including increased BDNF, improved vascular health and reduced inflammation. The honest hierarchy

If you ranked everything marketed for cognitive health by weight of evidence, aerobic exercise would sit at the top, and the gap to second place would be substantial. It is not close.

That is an awkward thing to publish on a site that discusses supplements, so let it stand plainly: no capsule in this category has evidence approaching what exercise has. Anything else you do for your brain is built on top of that foundation, not instead of it.

What the research shows

Hippocampal volume. The hippocampus is central to memory formation and it shrinks with age. Randomised controlled trials in older adults have found that aerobic exercise training increased hippocampal volume, effectively reversing a portion of age-related loss. That structural change is a striking finding for a behavioural intervention.

Executive function. Meta-analyses of exercise trials consistently show benefits for executive functions — planning, task switching, inhibitory control, working memory. Effects are typically modest but reliably positive across many studies.

Long-term risk. Large prospective cohort studies have repeatedly associated physical activity in midlife with lower rates of cognitive decline later. Observational evidence carries the usual caveats, but the direction is consistent across populations and decades.

Why it works

Several mechanisms operate together, which is likely why the effect is robust.

BDNF. Brain-derived neurotrophic factor supports neuronal survival, growth and synaptic plasticity. Aerobic exercise increases circulating BDNF, and it is among the most studied molecular explanations for exercise's cognitive effects.

How much you actually need

Public health guidance suggests 150 minutes of moderate-intensity aerobic activity per week, or 75 minutes of vigorous activity, plus muscle-strengthening on two days.

Two encouraging details sit behind that number. First, the largest marginal gains occur at the bottom of the curve — moving from nothing to something. Going from sedentary to a twenty-minute daily walk delivers more benefit than going from four sessions a week to five. Second, it accumulates: three ten-minute walks count.

Moderate intensity means you can talk but not sing. Brisk walking qualifies for most people. This does not require a gym, equipment or lycra.

What about resistance training?

The cognitive evidence base for aerobic exercise is larger, but resistance training has accumulating support of its own, particularly for executive function in older adults. It also maintains muscle mass and bone density, which protects independence — and independence protects the social and mental engagement that supports cognition.

The best answer is both. If you have to choose one, start with the aerobic component and add resistance work when the habit is established.

Starting from a realistic place

Advice to exercise is easy to give and hard to act on, especially when fatigue is the symptom you are trying to fix. Three principles that help:

  • Start absurdly small. Ten minutes daily beats a forty-minute plan you abandon in a fortnight. The habit is the asset; the duration can grow later.
  • Attach it to something fixed. After lunch, before the school run, on the way to the shops. Existing routines carry new habits.
  • Count walking. It is legitimate aerobic activity, it requires nothing, and the epidemiological evidence for walking and cognitive outcomes is genuinely good.
