Blueberry Polyphenols and the Ageing Brain: What the 2023 Trial Found

The short answer

A double-blind randomised controlled trial published in the American Journal of Clinical Nutrition in 2023 reported improved vascular function and cognitive performance in healthy older adults given blueberry polyphenols. Why blueberries keep appearing in cognitive research

Blueberries are dense in anthocyanins — the polyphenol class responsible for their colour — and anthocyanins do two things that make researchers interested in the ageing brain. They act as antioxidants, and they appear to influence vascular function, including the endothelial signalling that governs how blood vessels dilate.

Both mechanisms matter for cognition. The brain consumes roughly a fifth of the body's oxygen despite being a fraction of its mass, and it is unusually vulnerable to oxidative stress and to reductions in perfusion. An ingredient plausibly acting on both fronts is a reasonable thing to study.

The 2023 trial

The most directly relevant recent finding is a double-blind randomised controlled trial published in the American Journal of Clinical Nutrition in 2023, which reported improved vascular function and improved cognitive performance in healthy older adults given blueberry polyphenols.

Several features make this a stronger citation than most in the supplement world. It was randomised and double-blind, which controls for the expectation effects that plague nutrition research. It was conducted in healthy older adults rather than a clinical population, which is closer to the people actually buying cognitive supplements. And it measured vascular and cognitive outcomes in the same participants, which strengthens the mechanistic story rather than leaving it as speculation.

What the trial does not tell you

Three limitations worth being clear about.

It studied a defined polyphenol intake, not a capsule. The finding is about a quantified dose of blueberry polyphenols delivered in a controlled way. It does not transfer automatically to any product that lists "blueberry" on a label.

Anthocyanin content varies enormously. Fresh blueberries, freeze-dried powder, juice concentrate and standardised extract deliver very different polyphenol loads per gram. Processing method, cultivar and storage all move the number. A label saying "blueberry" without a quantity or standardisation tells you almost nothing.

One trial is one trial. A single well-designed study is meaningful, particularly alongside a broader body of observational work linking berry intake to slower cognitive ageing. It is not the same as a settled conclusion.

The observational backdrop

Beyond the randomised evidence, berry consumption has featured consistently in the epidemiological literature on cognitive ageing. Long-running cohort studies tracking dietary patterns have associated higher berry intake with slower rates of cognitive decline in older adults.

Observational findings carry the usual caveat — people who eat more berries tend to differ in many other ways, and no cohort study can fully separate the berries from the lifestyle. But when observational signals and a randomised trial point in the same direction, that convergence is worth something.

The dose problem in consumer products

Here is where the research and the supplement aisle part company. Trials specify polyphenol or anthocyanin content. Consumer labels almost never do.

When a supplement label lists "Blueberry" with no milligram figure and no standardisation, you cannot determine whether the capsule contains a research-relevant polyphenol load or a token amount included so the word can appear on the marketing. Both are possible and the label does not distinguish them.

This is not unique to any one product — it is close to universal in multi-ingredient cognitive formulas. But it means that a formula citing blueberry research is citing evidence it cannot demonstrate it replicates.

The cheaper alternative nobody mentions

Worth saying plainly: blueberries are a food, and eating them is not difficult. A cup of blueberries several times a week delivers a polyphenol load you can at least estimate, costs a few dollars, and comes with fibre and the general benefits of eating fruit. Frozen berries retain their anthocyanin content well and cost less than fresh.

That is not an argument against supplements containing blueberry. It is an argument for keeping the supplement in proportion: if blueberry polyphenols are a meaningful part of why a formula appeals to you, the food version is available, quantifiable and considerably cheaper.

The practical summary

Blueberry polyphenols have among the better recent evidence in the cognitive ingredient category — randomised, double-blind, in the right population, hitting both vascular and cognitive endpoints. That evidence attaches to quantified polyphenol intake, not to the word "blueberry" on a label. If a product states a milligram amount and a standardisation, you can evaluate it. If it does not, you are trusting rather than checking, and you may as well eat the fruit.
