The counterintuitive finding at the centre of this
If you want more nitric oxide, the obvious move is to take more arginine, because arginine is the direct substrate. Nitric oxide synthase converts arginine into nitric oxide and citrulline. Simple.
Except it does not work as cleanly as that. Oral arginine faces a substantial first-pass problem: a significant proportion is metabolised by the enzyme arginase in the gut wall and the liver before it ever reaches systemic circulation. You swallow arginine and a meaningful share of it never arrives where you wanted it.
L-citrulline takes the scenic route and gets there in better condition. Citrulline is not a good arginase substrate, so it largely survives the gut and liver, then converts to arginine in the kidneys. The net effect — repeatedly observed in pharmacokinetic work — is that oral citrulline tends to raise plasma arginine more effectively than an equivalent dose of oral arginine.
So why not just use citrulline?
Reasonable question, and formulators answer it differently. Three arguments for including both:
Different time courses. Arginine that does reach circulation is available immediately. Citrulline requires renal conversion first. Combining them produces a broader window rather than a single sharp peak.
Arginine has functions beyond nitric oxide. It participates in the urea cycle, in creatine synthesis and in protein synthesis. Supplying it directly is not solely about the nitric oxide pathway.
Cost and formulation. Arginine is generally cheaper per gram than citrulline. A blend achieves a given weight at a lower cost than pure citrulline would.
That last reason is worth naming honestly, because it is the least flattering and it is genuinely part of why blends look the way they do.
The forms you will see on labels
L-Arginine — the free amino acid, the baseline form.
L-Arginine AKG (alpha-ketoglutarate) — arginine bound to alpha-ketoglutarate, a tricarboxylic acid cycle intermediate. Marketed for improved uptake, though comparative evidence against plain arginine is not strong.
L-Citrulline (free form) — the straightforward version, most directly comparable to the research.
L-Citrulline Malate — citrulline bound to malic acid, typically in a 2:1 or 1:1 ratio. Very common in sports nutrition. Important detail: in a 2:1 preparation, only about two-thirds of the stated weight is citrulline; the rest is malate. Labels do not always make this clear.
L-Citrulline HCl — the hydrochloride salt, more soluble, with the salt contributing part of the stated weight.
That last point is the practical takeaway on forms: bound forms deliver less of the active amino acid per stated milligram than the free form does. A product listing 100 mg of citrulline malate contains less citrulline than one listing 100 mg of free citrulline.
Doses in the research versus doses in cognitive formulas
This is where expectations need calibrating. Studies examining citrulline's effects on blood flow, exercise performance and vascular function commonly use doses in the range of 3,000 to 8,000 mg per day. Arginine studies frequently use similar or larger amounts.
Cognitive supplement formulas typically include far less — often a few hundred milligrams across all forms combined. That is not necessarily useless, but it is an order of magnitude below the doses that generated the research these ingredients are cited for.
When a multi-ingredient formula lists arginine and citrulline at a combined few hundred milligrams, the honest framing is that these ingredients are present and contributing to the pathway, not that the product replicates the exercise-science literature.
What this pathway is actually well evidenced for
Nitric oxide precursor supplementation has its strongest evidence in exercise performance and cardiovascular contexts: blood pressure, endothelial function, exercise capacity. Those are legitimate, well-studied outcomes.
Cognition is a different question. The logic — better perfusion supports better brain function — is reasonable, and cerebral blood flow genuinely matters for cognitive performance. But studies measuring what oral arginine or citrulline does to memory or attention scores are far thinner than the cardiovascular literature. When a cognitive formula cites this pathway, it is borrowing evidence from an adjacent field.
Safety notes
Both amino acids are generally well tolerated, with gastrointestinal upset the most common complaint at higher doses. The interaction worth flagging: because both promote vasodilation, anyone taking antihypertensive medication, nitrates for angina, or PDE5 inhibitors should raise it with a pharmacist before adding a supplement containing either.
